1. Agreement to Participate and Assumption of Risk

I acknowledge that I have voluntarily chosen to participate in the practice of judo and general fitness activities at SouthWest Training Centre Pty Ltd.

I understand that the practice of judo involves physical activity and contact, which carries inherent risks, including but not limited to bodily injury, and disability. I voluntarily assume all risks associated with participation in these activities.

2. Medical Conditions

I declare that I am physically fit and have no medical conditions that would prevent my participation in judo or that would be adversely affected by my participation. I agree to disclose any medical conditions to the club prior to participating.

3. Judo WA Membership & Insurance Coverage

I acknowledge that as a condition of participation, I am required to become a member of Judo Western Australia (Inc.), which provides insurance coverage, including personal accident insurance. I understand that more information about this coverage can be found at Judo Australia Insurance.

4. Release and Indemnity

In consideration of being permitted to participate in the activities at SouthWest Training Centre Pty Ltd, I agree to release, indemnify, and hold harmless SouthWest Training Centre Pty Ltd, its directors, officers, employees, coaches, volunteers, and agents from any and all claims, demands, actions, or causes of action, which may arise out of or are in any way connected with my participation in these activities, including any claims arising from negligence.

5. Parental/Guardian Consent for Minors

If the participant is a minor, a parent or guardian must read and sign this agreement. As a parent or guardian, I consent to the minor’s participation in the activities and agree to the terms outlined in this waiver on behalf of the minor.

6. Code of Conduct and Judo Values

I agree to adhere to the values of judo known as 'The Great 8': Courage, Respect, Modesty, Friendship, Honour, Sincerity, Self-control, and Politeness. I understand that these values are integral to the practice of judo and the community at SouthWest Training Centre Pty Ltd.

7. Photography/Video Consent

I consent to the use of photographs and videos taken during training sessions and events for promotional and training purposes by SouthWest Training Centre Pty Ltd.

8. Membership Terms

  • No-Lock-In Contract: All memberships at SouthWest Training Centre Pty Ltd are offered on a no-lock-in contract basis.
  • Cancellation: Members must provide at least 1 month's notice in order to cancel their membership. Refunds will not be provided for processed payments where members provide late notice of cancellation.
  • Freezing Membership: Members may pause their membership by providing at least 2 weeks' notice and must indicate an unfreeze date when doing so.
  • Fee Adjustments: SouthWest Training Centre Pty Ltd reserves the right to periodically increase membership fees. Members will be provided with at least 30 days' notice of any fee changes.
  • Christmas and New Year Closure: All existing fees take into account a 3-week closure for Christmas and New Year.

9. Duration and Validity

This waiver and release shall remain in effect as long as I am a registered member of SouthWest Training Centre Pty Ltd.
